How to Get Vaccinated
Walk-ins: Bring a photo ID and any insurance cards. You'll complete a consent form on site. Most insurance plans cover the flu vaccine; some do not cover the administration fee. Patients with appointments are taken first.

Before Your Appointment
Please review our Flu Vaccine Information Sheet and HIPAA Privacy Notice. You'll be asked to sign a consent form verifying you've read both. You may also print the Flu Consent Form, complete it at home, and bring it with you.
On the Day of Your Appointment
If you booked online, arrive a few minutes early. You'll complete a consent form unless you bring a printed, completed one. If you walk in, you may need to wait up to an hour. You'll review the Vaccine Information Sheet, complete consent, and provide your insurance information.
After Your Appointment
Please remain in the pharmacy for at least 10 minutes so we can monitor for any immediate side effects.
